I have attached a copy of the claim form you can use which includes full terms and conditions, please return these forms to me via email and I will process your free of charge stock via your wholesaler.

Please note:
💉 Any dog or cat not currently receiving treatment with Librela or Solensia can take advantage of their first dose of treatment for free💉
🐕 There is space for six doses per claim form but there is no limitation on the number of doses each practice can claim so multiple forms can be submitted.
🐈 As Librela and Solensia come in packs of two vials, I will need to allow for this when processing claims.

The initiative will run for a limited period until the end of September, so please do not delay, take advantage of the offer and help us to ensure more dogs and cats get more days of play!
